To help keep our community safe, please complete this confidential form. Your emergency and medical details will only be accessed by the TRYBZ team or an official event host in the event of an emergency during a TRYBZ activity.

Most TRYBZ events are low-risk, but some activities (e.g. fitness sessions, movement workshops, hikes, dance, outdoor activities or sports) may involve physical exertion or increased risk. This information helps us act quickly and appropriately if something unexpected occurs.

You only need to complete this form once. If your details change in the future, you can update your responses using the same email address you used to submit the form. A copy of your completed form will be sent to your email address for your records.
