(The appointment is only for those Citizens and Special Residents aspiring to process CID/SR card from the Department of Civil Registration and Census. Use the link provided at the end of this page for the appointment):
1. Kindly use the online Appointment System to decongest crowd and reduce waiting time to process the Citizenship Identity/Special Resident Card.
2. Provide your correct name, Citizenship Identity/Special Resident Card Number and mobile number. Please bring all the required documents when you come to process Citizenship Card/Special Resident Card. For detailed info visit https://dcrc.mohca.gov.bt

3. Kindly report to process card as per the Time Slot (09:00 AM to 10:30 AM; 10:30 AM to 01:00 PM; 02:00 PM to 03:30 PM and 03:30 PM to 05:00 PM) you have secured to avoid overcrowding for COVID safety and optimize your time for other purposes.
4. Follow the TOKEN Number while availing services. You will be provided a TOKEN from the Appointment and Information Counter located at the Ground Floor. Please note that TOKEN is being issued only for those registered in the Appointment System.
A. NEW CID/SR CARD (Age 15-17 Years)
You can process New CID/SR card from the Dzongkhag/Thromde you are currently residing with following documents and fees:
I. Duly completed application form BCRS-CID/SRC-01 (Download Application Form: https://dcrc.mohca.gov.bt/wp-content/uploads/2022/02/BCRS-CID_SRC-01-3.pdf
II. Produce ORIGINAL Health Card/Mother & Child Health Handbook and if you have lost/misplaced it you must obtain the Seal and Signature of concerned Gup/Thromde Representative/Thromde Tshogpa in the space provided for this purpose in the application form. The photo affixed on the application form must be stamped with OFFICIAL SEAL of the concerned Gewog/Thromde Tshogpa if you are applying for CID/SR card with signature/endorsement of the LG officials.
III. Certificate/Dratshang ID card for Monks and Nuns.
IV. Card fee Nu. 100 (Cash)
B. NEW CID/SR CARD (Age 18 Years and above)
You can process New CID card from the Dzongkhag/Thromde where you are currently residing with following documents and fees:
I. Duly completed application form BCRS-CID/SRC-02 (Download Application Form : https://dcrc.mohca.gov.bt/wp-content/uploads/2022/02/BCRS-CID_SRC-02-3.pdf
II. Must obtain signatures of your Gup, Dzongkhag CRCO/Thromde CRCO and Dzongdag/Executive Secretary in the space provided for this purpose in the application form. The photo affixed on the application form must be stamped with OFFICIAL SEAL of the concerned Gewog/Thromde Tshogpa.
III. Statement of Gup and Tshogpa if you have attained 25 years and reporting to process card for the firs time.
IV. Certificate/Dratshang ID card for Monks and Nuns.
V. Card fee Nu. 100 (Cash).
C. RENEWAL / REPLACEMENT OF CID/SR CARD
You can process Renewal/Replacement of CID/SR card from the Dzongkhag/Thromde where you are currently residing with following documents and fees:
I. Duly completed application form BCRS-REP-01 (Download Application Form, Download Form: https://dcrc.mohca.gov.bt/wp-content/uploads/2022/01/Replacement-of-CID_SR-Card-Application-Form-BCRS-CID_SRC-REP-01.pdf
II. Original CID/SR card (Renewal and Replacement).
III. One Passport-size photograph (For Lost application).
IV. Monks, Nuns and other religious persons must produce certification from concerned Head of Religious Institution for change in occupation to/from other occupation. Must produce a verification report of Royal Bhutan Police if you have lost the Original Card.
V. Card fee (Cash):
- Nu. 100 for Renewal
- Nu. 300 for Replacement
- Nu. 400 for Lost
