Thank you for taking the time to fill out this form! We are a group of disabled volunteers working to get free masks to you.

As a general notice, this form is for individuals / families / households in DFW, TX to fill out to receive free masks! For organizations, educators, events, please check out our organization mask request form.

We currently are only offering delivery in sending masks to individuals/families/households the DFW metroplex and surrounding areas (generally about a 100 mile circumference distance with the Irving/Arlington area serving as the center).

*we are currently only able to ship out masks to you once every 30 days*

We provide high quality face masks (N95, KN95, KF94, etc.) to those in need in our local DFW community. N95's are high-quality respirator masks, the style recommended to best protect yourself and others since the onset of the Omicron variant of COVID-19. They are NIOSH approved and individually wrapped. Masks can be reused until soiled or damaged -- having several masks will allow them to be rotated to extend their lives.
