PASA Data to Inform Survey Work
In a collaborative effort between the Colorado Department of Public Health and Environment (CDPHE) and the Colorado Department of Health Care Policy and Financing (HCPF), data collection efforts are underway to best inform our ongoing survey work of Program Approved Service Agencies (PASAs).  As such, please complete and submit the following google form by July 30, 2021 with information about your facility’s current number of persons receiving services, information about your facility's practice of maintaining and updating the roster, services currently offered and your facility’s Program Provider Identification Number (PPID).

What services does your PASA currently provide?
How many individuals does your PASA presently serve, total?
Describe the process by which your facility maintains, updates, and makes available an up-to-date roster, as required.
What is your PASA's Program Provider Identification Number (PPID)?  This is the per location, unique identifier issued by Gainwell Technologies (formerly (DXC) issued for each Medicaid provider enrollment.  Please list any and all PPIDs below.  Please note, the PPID is not to be confused with the National Provider ID (NPI) that is issued by the National Plan and Provider Enumeration System (NPPES), and not every HCBS provider may have a PPID.
