Welcome Strategic Plan Champion,

The purpose of this form is to promote and encourage purposeful and sustainable communication between SIP Champions and EOEL. EOEL is interested in learning about specific aspects from SIP champions through the duration if the SIP lifetime and document ways champions may need support.

You will be asked a series of six questions for each objective. Based on the collaborative nature of SIP process, groups of champions will need to choose an individual member to fill this form based on collective feedback.  Most ideal situation is for champion groups to engage with each other and answer these questions collectively.  Thus champions are encouraged to set up an engagement opportunity to collect information from various members to fill this form.

We would like to collect this information on at least an annual basis for each SIP.  If the information can be provided more than once a year, we encourage champion groups to fill the form more than once a year if capacity and resources allow.

This information will be incorporated into a larger annual update to Hawaii's Early Learning Board and to the public at www.earlychildhoodhawaii.com.  This information will also help EOEL support state plan monitoring and share the important work of strategic plan champions which contributes to larger collective system goals.
