Northumberland County Reentry Services Assessment Survey
Dear Northumberland County Community Service Provider,

Central Susquehanna Opportunities, Inc. (CSO) received a grant from the Pennsylvania Commission on Crime and Delinquency to develop a plan for reentry for individuals who are being released from the county prison.  As part of this process, a needs assessment is being conducted to identify the strengths and weaknesses of the services currently offered in the county.

As a service provider in the community, your input is valuable to this process and we would appreciate your time in completing this brief attached survey. The responses to the survey will provide information about which services the county provides and where gaps exist.
We also want to obtain your perspective, as a service provider, on steps, the county can take to better assist individuals reintegrating back into the community.

Your participation is voluntary; you can choose not to answer any questions you are not comfortable answering. The results gathered are anonymous; no one can identify responses from any agency. There is no direct benefit to u or your agency for participating. The results from the survey will aid us to identify and develop services to assist individuals released from prison.
