What is a Patient Participation Group (PPG)
It is a national initiative that is aimed to strengthen the relationship between patients and their practices.

At Crossroads we are looking to work with the practice to engage with patients, plus other local organisations such as community groups, to help improve the facilities we currently have.

This may be achieved by conducting patient surveys, promoting talks on health & wellbeing, fundraising for equipment, running stalls at local events and communicating with residents.

We are only in the infant stage of forming the group and a committee. Any support from you would be very welcome and individuals are needed to represent a cross section of the patients registered at this practice.

There are several ways you can be involved, or you may simply wish to be kept up to date with what you PPG is up to.
Please complete your contact details on the next page and a member of the group will be in contact with you shortly.
