All registered participants, attendees, vendors and guests of the in-person NACSA Annual Conference (NACSACon) in Minneapolis, Minnesota are required to be “Up to Date” in their vaccinations against COVID-19 unless they are eligible for an exemption. This policy is for the safety and welfare of our attendees and based on guidance and recommendations issued by the Centers for Disease Control and Prevention (CDC) and other public health authorities and applies to all events and meetings in connection with NACSACon.

To request an exemption from the COVID-19 vaccination requirement due to a health circumstance or sincerely held religious belief, please complete the required form below. A NACSA staff member will follow-up to let you know your request status.

Individuals granted an exemption must still provide proof electronically that they have tested negative for COVID-19 via a PCR COVID-19 test (verified by a healthcare provider) no more than 48 hours prior to their arrival at the NACSACon Registration Area.  Attendees will be responsible for arranging and paying for any such tests.
