In addition to the sports physical completed by your student athlete's physician, please find the three documents that GCS requires before a student participates in our Varsity and Junior Varsity sports program.  Please complete one set of forms for each of your student athletes after reading our Parent/Student Athletic Handbook, which can be found on the GCS website under the "Family Hub" menu.  

The physical must be completed after May 1 for the upcoming school year.  The physical will cover the athlete for the entire school year.  Any athlete who has not turned in a completed, signed physical form may not play or practice.
