Please use the form below to report any violations of our safe-space policy that you’ve experienced or have witnessed. You may choose to either remain anonymous or provide your name and email. FCG is not collecting the email addresses attached to this form.

All reports, follow-ups, discussions, and courses of action will remain confidential. If a staff, Board Member, organizer, coordinator, artist, intern, or volunteer is reported as the harasser they will not be part of the discussions and decision-making processes with regard to courses of action.

We want to ensure that every person feels safe and is treated with dignity at our events and in our spaces. You should not feel bad or guilty for reporting an incident where you or another individual was made to feel unsafe or unwelcome.
