Doors of Compassion will be held April 27, 2019. Please fill out the following registration form, indicating your interest in being an Elite Chef for this one-of-a-kind fundraising event. If you have any questions, please contact events@rmhcdayton.org.

Bu submitting this form, the named chef is confirming participation in the Doors of Compassion event on April 27, 2019. Ronald McDonald House Charities of the Miami Valley will not be held liable for possible bodily injury/property damages before, during or after the event. You hereby irrevocably grant to RMHC, its affiliates and others authorized by RMHC, the absolute right and permission to use your name, biographical information, voice, actions, photograph, video, likeness, appearance, interviews, evaluations, opinions, and any other information contained in, derived from or otherwise obtained by RMHC in connection with your participation, in any manner or media whatsoever, now known or hereafter created, worldwide, for purposes of advertising, social media, publicity, trade, or promotion. You acknowledge and agree that you shall have no right of approval, no claim to any compensation, and no claim arising out of any use of the Materials. Participation includes chef services for the evening, any prior preparations for the evenings’ dinner, as well as prior communications with the host of the chef’s assigned host home. Confirmed basic food items will be revealed closer to the event. Any other expenses are to be picked up by the chef. Food preparations including utensils, cookware, seasonings, etc. are the responsibility of the chef. All chefs are invited and encouraged to attend the After Party compliments of the Doors of Compassion Steering Committee. Chefs will receive a thank you letter for tax purposes, as well as recognition via media related to the Doors of Compassion event, in the event program and on www.RMHCdayton.org. *
