Lemonade Day 2019-20 Agreement and Materials Order
Greetings new and returning Community Partners of Lemonade Day Northeast Ohio! You are invited to participate in Lemonade Day Northeast Ohio at no cost due to the generous support of Burton D. Morgan Foundation, The Martha Holden Jennings Foundation, The Veale Foundation, Geis Foundation and Hudson Community Foundation.

Please provide the information below which includes your digital agreement to the terms of our Memorandum of Understanding and your Lemonade Day materials order.

Upload your classroom roster at the time of registration to help us with data collection for our grant reporting. If you do not have a roster of the actual students you are serving (because it's too early to know, for example), please submit a sample roster from a previous program or a tentative roster of names.

Participant Information & Materials Order
Lemonade Day curriculum is appropriate for students in grades K-12. Two workbook versions are available: Youth and Teen. Youth edition is typically used by our elementary students and teen edition by our middle and high school students. We also have a third option: Lemonopolis. This is an online version of the curriculum in the format of a learning game. Lemonopolis is supported by My Journal, a booklet in which students can record answers and ideas. Lemonopolis is targeted at grades 4, 5 and 6. There is also a Spanish language version of the workbook and mentor guide available in a PDF file for you to print.
Please indicate below how many sets of each type of materials you would like to order. Printed materials orders exceeding 100 sets for returning partners will be shipped directly to the address provided above. New partners will receive materials by delivery when training session is scheduled.

Lemonade Day Lemonopolis Game & My Journal Order
Number of individual licenses for online curriculum accompanied by printed booklet. This version of the curriculum is geared towards students in grades 4, 5, and 6. Lemonopolis also includes option to print your own My Journal from PDF.

Materials Pick-up Location

Returning partners ordering 100 or more sets of printed materials will receive them by direct shipment to the address provided above. New partners will receive a scheduling request for us to visit to provide training; materials will be delivered when training occurs.

Memorandum of Understanding - Commitments & Best Practices *
Promotional: I agree to promote Lemonade Day throughout my organization and in the community; To provide Lemonade Day promotional and informational materials in my organization; To submit my organization’s logo to the Regional Director in three formats: vector eps color, black and white, and jpeg. Registration and Distribution: To provide and commit to a registration goal (above) so backpacks can be ordered and purchased; To officially register youth in my organization through registration forms and/or spreadsheets; To partner with the Regional Director to create a plan for backpack delivery and distribution. Operational Best Practices: To identify key people who will help make this program a success; To identify a program coordinator for my location; To hyperlink Lemonade Day on my organization’s website and social media sites, if appropriate; To develop a timeline for implementation of Lemonade Day in my organization’s program; To encourage my employees to participate in Lemonade Day; To provide information about stand locations and times to the Regional Director. Program Quality: To teach Lemonade Day lessons in my organization’s programs; To encourage youth who participate to complete the program by setting up a lemonade stand; To report to the Regional Director the number of lemonade stands opened by my youth; To collect business results information from youth who set up lemonade stands; To share photos and special stories of how Lemonade Day impacted youth in my organization.
Memorandum of Understanding - Benefits of Lemonade Day *
Promotional benefits: Your organization logo included on website of Lemonade Day Northeast Ohio; Your organization website hyperlinked on website of Lemonade Day Northeast Ohio; Potential PR opportunities for your organization’s participation in Lemonade Day. Materials/Resources: Lemonade Day backpack, Entrepreneur Workbook, printed Mentor Guide for each registered youth, or Lemonopolis license and My Journal workbook; Teacher Guide for staff/mentors; Support and training if necessary for the appointed Lemonade Day educators in your organization.
Memorandum of Understanding - Responsibility Statement *
I understand that the Young Entrepreneur Institute at University School, the organizers of Lemonade Day Northeast Ohio, are not and will not be responsible for supervising my students. I also understand that certain municipalities may have certain health and safety requirements so that my students can participate in Lemonade Day. I agree to comply with these requirements. (check your city's website for local requirements.) I also understand that certain municipalities must be provided the name of the person who will supervise my child and the exact location of my child's lemonade stand ahead of time.
Memorandum of Understanding - Mutual Indemnity Agreement *
I agree to release, indemnify, defend and hold harmless University School, and anyone associated with it or Lemonade Day from any and all claims for personal injuries or property damage resulting from my student’s participation in Lemonade Day, even if such injury is caused by the negligence of them.

I understand the Lemonade Day workbook and supporting materials are copyrighted content and may not be reproduced fully or in part without permission.
Memorandum of Understanding - Photo Release Agreement *
I grant to Lemonade Day Northeast Ohio the right to take photographs and video of participants from my organization in connection with Lemonade Day events. I authorize Lemonade Day Northeast Ohio and its partners to use and publish the same in print and/or electronically. I agree that Lemonade Day Northeast Ohio and its partners may use such photographs and video for any lawful purpose, including for example such purposes as publicity, illustration, advertising and Web content. I grant the same permissions for photographs or videos my organization provides to Lemonade Day Northeast Ohio and its partners.
Loan Board Procedure (optional)
Many of our partners conduct a "Loan Board" event in which student teams present business plans and loan requests to a panel of adult volunteer Loan Officers. Usually the event is conducted in the classroom. Loan Board is unique to Northeast Ohio and is not described in the workbook materials. If you choose to add this component, we are happy to help identify volunteers. Thanks to a 2014 grant from KeyBank, we have established a loan fund from which your organization may borrow, interest free. In order to participate, please answer the questions below.

Each student team may borrow up to $50 from the YEI loan fund, unless the team provides sufficient justification for a larger loan amount, as judged by the loan board or teacher/mentor. Approval of loan requests is given by teachers who then submit total loan request to YEI.
