Remote Lunch Order Form
Please fill in all fields to order breakfasts and lunches while remote learning. The USDA has approved federal free lunch for all students in the school district through the end of the school year. Pick up will be on Wednesdays from 10:00AM-12:00PM at the FELC, Knollwood, or Brookside High School. If you are unable to pick-up at this time, please leave a message in the comment back at the bottom and a student services worker will assist you with scheduling an alternative time.
If you are a hybrid student and are in-school, pre-ordering food for your in-school days is not needed. Breakfasts and Lunches will still be available at school for when your child is learning in-person at school.
This order form is for when you are learning remotely and would like to receive a breakfast & lunch while learning from home.
Note! This form is for pickups on Wednesday October 28 and Wednesday November 4.

Please select one of the options listed below. Breakfasts & Lunches will be packaged with instructions. There will be a variety of foods that can be eaten the same day, refrigerated, or frozen and reheated throughout the week.
I am a full remote learner and will be picking up 5 breakfasts and lunches on Wednesday
I am a hybrid student and will be picking up 1 breakfast and lunch on Wednesday (due to in-person school on MTRF))

Breakfasts/Lunches are available for pick-up between 10:00AM-12:00PM. Please choose one of the following pick-up locations (you can pick-up at any of the following schools even if you don't attend school at that building):
Knollwood- front of the school in the bus circle (there will be a staff member there to assist you)
Forestlawn ELC- gym doors in the back of the building (there will be a staff member there to assist you)
Brookside HS- event entrance in the back of the building (there will be a staff member there to assist you)
