Living Will Worksheet
A Living Will is a legal document that allows you to declare, in the event that you are certified by two licensed physicians that you have become permanently incapacitated, whether you would like to continue artificial life support, or that you would like to have artificial life support withdrawn.

This document also allows you to name a person or people to be notified should you become permanently incapacitated and to declare, in the event of your death, whether you would like to donate some or all of your organs.

This Worksheet should be completed for each person that would like to have this document prepared.
