I the undersigned, acknowledge and fully understands that each participant will be engaging in activities that involve risks of serious injury that may result from their own negligence of others, the rules of play or the condition and that there may be other risks not known to us or reasonably foreseeable at this time. The undersigned assumes all of the foregoing risks and accepts personal responsibility for any damages following an injury should one occur. Releases, waives, discharges and agrees to hold harmless and convents not to sue New Jersey Falcons their respective administrators, leases and lessors' of the premises used to conduct the event from demands, losses, or damages on account of injury. I give consent to administer first aid and emergency transport to the nearest medical facility to New Jersey Falcons, in the case of an injury. I understand that the registration of my child does not necessarily guarantee that he/she has been accepted into the program until the final rosters are determined. I understand that players are admitted in to New Jersey Falcons on a conditional basis pending acceptance on a first-come, first-served and/or a skill-based criterion. All fees (if applicable) paid to New Jersey Falcons are nonrefundable.
