UYM Surf City Sign up
This form is to only sign up a student for Surf City, August 2 - 7, 2020. This trip is only open to students who are in 6th-8th grade during the 2020-21 school year. The total cost of the trip is $460/student. A deposit of $100 to sign up for this trip is due by March 15, 2020. Final payments will be due by July 1, 2020. Please complete one form for each student you are signing up.

There will be a parent meeting about Surf City on Sunday, March 15 following both church services (10:30am and 12:30pm).
