Please complete this if you are a Remote student and would like to transition back to Traditional Learning option at the start of Q2 on October 28, 2020.

If you have multiple students, please complete the form for each student separately.

Counselors will reach out to you either by phone or email to confirm or to discuss any necessary schedule changes.

Students should continue to attend via remote learning. If a remote learning student shows up to school before October 28, 2020 they will be sent home.

If you will need to ride a school bus, please direct all transportation questions to the district transportation department 850-469-5603 for Richard Lopez, route manager.
