RELEASE OF LIABILITY, WAIVER OF CLAIMS, ASSUMPTION OF RISKS AND INDEMNITY AGREEMENT. BY SIGNING THIS DOCUMENT YOU WILL WAIVE OR GIVE UP CERTAIN LEGAL RIGHTS, INCLUDING THE RIGHT TO SUE OR CLAIM COMPENSATION. PLEASE READ CAREFULLY.

TO: Cumberland Community Schools Society (CCSS), Weird Church, the Corporation of the Village of Cumberland, and all owners or occupiers of venues or premises Activities (as defined herein) take place, and their respective directors, officers, employees, members, volunteers, and assigns (all of whom are hereinafter collectively referred as “the Releasees”).

AWARENESS AND ASSUMPTION OF RISK
I am aware that participation in this program involves many risks, dangers and hazards including, but not limited to: contracting COVID-19 or other illness from CCSS representatives or other participants, and negligence on the part of CCSS and our community partners. I freely accept and fully assume all such risks, dangers and hazards and the possibility of personal injury, death or loss resulting therefrom.

In consideration of CCSS agreeing to my participation and/or my child/ward’s participation in this program, I hereby agree, on behalf of myself and/or my child/ward to waive any and all claims that I have or may in the future have against CCSS and to release CCSS from any and all liability for any injury, loss, damage or expense, including death, that I may suffer or that my next of kin may suffer, as a result of my participation in this program, due to any cause whatsoever, including negligence, breach of contract, or breach of any statutory or other duty of care on the part of CCSS. I understand that negligence includes failure of CCSS to take reasonable steps to safeguard or protect me from the risks, dangers and hazards associated with this program including contracting COVID-19.

I CONFIRM THAT I HAVE READ AND UNDERSTOOD THIS RELEASE AGREEMENT ON BEHALF OF MYSELF AND/OR MY CHILD/WARD, PRIOR TO SIGNING IT, AND I AM AWARE THAT BY SIGNING THIS RELEASE AGREEMENT I AM WAIVING CERTAIN LEGAL RIGHTS WHICH I OR MY HEIRS, NEXT OF KIN, EXECUTORS, ADMINISTRATORS, ASSIGNS AND
REPRESENTATIVES MAY HAVE AGAINST THE RELEASEES.
