For students participating and/or completing a driver's education program, a Verification of Compliance (VOC) letter is required by the DMV in order to process a student's driving permit and/or driver's license.  VOCs may be requested by students and/or their parent using this form.

Please be advised that student information in Infinite Campus MUST MATCH the student's home address when requesting this form.  The student's information in Infinite Campus will be used by office staff to process the VOC request.  If the information in Infinite Campus and on this form DOES NOT MATCH, please email the Registrar - Chrissy Beckner (cbeckner@bk.org) to update your information.

The DMV requires an actual signature and school stamp that is not accepted via fax or e-mail/PDF.
