Summer 2018 Tech Form
Please complete this from prior to May 25 if you have a specific TECH issue that needs addressed. If the problem is with your laptop or desktop, please attach your login and password to the computer--or send to me via email. I am a staff of ONE but I will do my best to accommodate. This form is for updates and maintenance requests--NOT FOR PURCHASES. If you are responsible for a Tub or Cart--please check every device and notify me immediately if repairs need to be made--as those need to be sent in and I'd like everything repaired and returned by the end of June.
