I am submitting this application for membership in Greater Ashmont Main Street, Inc., for myself or as a representative of the business or organization named above. In doing so, I certify that I am at least eighteen years of age and that I support the purpose and vision of Greater Ashmont Main Street, Inc., or represent a business or organization that supports that purpose and vision. This application is not complete until annual membership dues are received. Current dues rates are $20 for individuals and $50 for business (including non-profit organizations). Annual and event-specific corporate sponsorship opportunities are also available.

In the case of new members, I understand that the Board of Directors will consider this application at its next regularly scheduled meeting. In the case of renewals, I understand this form is meant to confirm/update my membership information and areas of interest.
