**PARENTS AND SENIOR, PLEASE READ THOROUGHLY BEFORE SUBMITTING. By clicking submit, I understand that if I am chosen as a Gurley Girl Senior Rep by Madison Gurley Photography, I will be required to use Madison Gurley Photography as my senior photographer and will contractually agree to not use any other photographers for the duration of the senior rep program for any reason. I also understand that this application will be reviewed by
Madison Gurley
and is not guaranteed acceptance into the senior rep program. In addition, I understand that, if chosen, as a senior rep, I am required to pay the full, non-refundable membership fee by August 31, 2023, as well as purchase products from my individual sessions by May 31, 2024, but will have an opportunity to earn print credits towards my artwork purchase through referrals. I understand that submitting the application does not secure my spot on the team, and should I be selected as a team member, I will receive communication and respond to Madison by middle of August 2023.
By submitting this application, you are agreeing that you have discussed your application with your parent(s)/guardian(s) and are agreeing that if chosen, you will be required to pay the membership fee payment of $1875 at time of acceptance (by August 31, 2023). If payment cannot be made or your parents are not in agreement to your participation, you will not be able to participate as a Gurley Girl Senior Rep, but I would love to discuss other senior packages with you to determine what best fits your budget.
