North Ogden Junior High School: Absence Verification Request Form

North Ogden Junior High parents/guardians may use this form to excuse an absence for their student, or a range of absences that does not exceed six (6) consecutive school days. If a parent/guardian is excusing an absence for more than one student, a separate form must be submitted for each student.

There is no need to call the school after submitting this form. Attendance records will be updated in the system within 24 hours. Please do not call the school regarding the absence before this time. The Attendance Office will contact you only if there are any issues verifying the absence request.

Prior to submitting this form, please review the following information:

-A pre-arranged absence is notice given at least one (1) school day in advance.

-An absence must be verified within five (5) school days following the absence.

-The five (5) school day limit does not extend beyond the end of each term. All requests for an absence to be excused must be submitted by the last day of the term.


-A valid excuse must be provided in accordance with district policy (Section V.C.2) in order for an absence to be excused.

-If an Absence Verification Request Form is submitted by a student or from an email address that is not verified in PowerSchool, the absence will not be excused. In this case, the absence must be addressed by contacting the North Ogden Junior High Attendance Office or by visiting the main office in person.


PLEASE NOTE: THIS FORM MAY NOT BE USED TO CHECK A STUDENT OUT EARLY

To check a student out early from school, a parent/guardian must enter through the main entrance and present a valid form of identification at the main office window.
Any student arriving late to school or returning after leaving campus must check in at the main office window.

For absences extending beyond six (6) consecutive school days, an Extended Absence Form must be completed. This form may be picked up from the Attendance Office and must be submitted prior to the scheduled absence.
