ENROLMENT: Enrolments will be confirmed in writing prior to the course commencement date and are subject to: (1) receipt of the completed training application form and; (2) payment prior to the course date, or receipt of purchase order from participant or from approved company’s Account or Finance Department.

FEES: Course fees are payable in advance and cover the cost of training materials, equipment and lunch/refreshments. Course fees are shown in Malaysian Ringgit excluding GST and are subject to change without notice.

FEES: By enrolling in this course, you agree that the course fees cover the expenses for attendance at five days of physical meeting classes, provision of hard-copy and softcopy materials, access to online training information references, as well as examination fees. Please note that the fees do not include charges for resit examinations.

PAYMENT: Payment for participation in this training program must be made via online transfer, cheque, or CDM/ATM transfer, with detailed instructions provided in the quotation or invoice.

PAYMENT THROUGH HRD CORP: Payment made via HRDF claimable process (if applicable) is subject to compliance with HRDF procedures outlined in the quotation or invoice; or otherwise, the payment shall be processed through the normal payment process mode.

CANCELLATION: Requests for cancellation must be received in writing and acknowledged by OSHISIS Solution PLT. Full refund available for cancellations received at least 5 working days prior to course commencement. The full course fee will be charged for cancellations received under 5 working days. Suitable substitutes are welcome.

TRAINING MATERIAL: By accessing and utilizing OSHISIS training materials in softcopy and referencing provided weblinks, participants acknowledge that any request for hardcopy materials will incur an additional cost of RM300 per unit, except for the purpose of assignments, exercises, discussion, group activities, or materials deemed necessary by the training provider.

GENERAL: OSHISIS Solution reserves the right to change course schedules, discontinue courses, modify course content, limit class size and cancel courses.

DISCOUNT: Discounted rate is offered for group registration in one online form or the person attending the MSPO Auditor Program from 2018 to 2021 organized by OSHISIS (former known as OSH ISIS Integrated Solution).

HRDF CLAIM: By registering for this training program under the HRDF (SBL-Khas Scheme), you agree to incur a 6% additional fee to cover the service fee to HRD Corp and administrative processes.

CERTIFICATE OF ACHIEVEMENT: By sitting an examination after completion of 5 days classes, you agree that certificates will be issued accordingly: participants who pass the examination will receive a certificate reflecting their achievement, while those who do not pass will receive an attendance certificate. Participants failing the examination will be granted one opportunity for a free re-sit; otherwise, they will receive the attendance certificate only.

CERTIFICATE: Upon successful completion of the training program, an electronic certificate (E-Certificate) will be issued to the participant via email. The E-Certificate will be sent to the email address provided by the participant during registration or as updated later. Participants who require a hardcopy of the certificate may request one by contacting the training provider via email or phone. The request should include the participant's full name, the training program completed, and any other necessary details. A fee of RM50.00 (Malaysian Ringgit) will be charged for each hardcopy certificate requested. This fee covers the printing, handling, and postage costs associated with sending the hardcopy certificate to the participant.
