This form will serve as the official registration form for the 9th Annual Monarch Invitational.  Please mail your check for the $75 registration fee payable to NHS Band Boosters soon after you apply online. Your registration will be considered complete once we receive your check. REGISTRATION CLOSES AFTER FRIDAY, SEPTEMBER 7, 2018.  The date of the competition is October 6, 2018.  An additional form will be distributed on Saturday, September 8 for additional details.  THIS IS ONLY THE REGISTRATION FORM.

Performance order (within each class) are to be determined by registration order.  
Class sizes are to be determined by number of WINDS only.
