You must pull out by the 20th of the prior month that you would like to stop. (For example, if you want to stop starting the month of February, you need to fill this form out by January 20th in order for your credit card to not be charged.)

We must receive this form by the 20th of the month prior to avoid a monthly charge. For example, if you do not want to continue classes in December, we would need the form by November 20th.

If we receive this form after the 20th, your card will be charged for the following month on the 1st of the month.
