2020-2021 Re-Opening Survey
As you are aware, COVID-19 has had a significant impact on how we deliver our instructional program to meet the needs of our students. With guidance released from NYSED and the NYSDOH, districts are required to create plans that include remote (on-line) instruction, in-person instruction and a hybrid model (a combination of in-person and remote instruction). We are asking you to complete a survey for each child so we can utilize this information to continue our planning for the 2020-2021 school year. Your responses to the survey will be considered in our planning and does not bind you to the choice selected. Thank you.
Please provide us with the following student information so that we can properly prepare for the 2020-2021 school year.
What is your student's name?
What grade will your student be entering the 2020-2021 school year?
What is the working email and best phone number for parent notification for this student?
Forms of Instruction (Remote or In-person)
There are mandatory requirements for in-person instruction established by the NYSED. Some of the requirements for the in-person instruction are: face coverings for students, staff and visitors in all buildings and on buses (will be provided if your student does not have one); daily temperature checks; social distancing (6 feet) in all District buildings and classrooms; practicing good hygiene, including frequent hand washing; and adjustments to our daily schedules. Our remote instruction will look differently this year as well, mainly that all students will be required to follow a daily schedule.
