Please use this form to register for the Mike Hunger Clinic on Saturday, July 13th 9:00am to 4:00 pm at Altius and Sunday, July 14th from 9:00 am - 4:00 pm at Midwest Twisters - Hartland. Cost is $25 per coach & $50 per athlete. Each coach may bring 3 athletes... maximum of 6 athletes per club. ONE FREE coach for every 3 athletes registered. Please be sure to follow Safe Sport travel guidelines.
Coaches and athletes need to bring a bag lunch as there may not be time to leave the facility for lunch.

All participants MUST have a current USA Gymnastics membership. This includes both the coaches and the athletes. Please follow Safe Sport travel guidelines.
3 athletes ($150) + coach (FREE) = $150 total
2 athletes ($100) + coach ($25) = $125 total
1 athlete ($50) + coach ($25) = $75 total
1 Coach ($25) + 0 Athletes ($0) = $25 total
I understand that payment is due whether the coach/athlete(s) attend or not.
I understand that all coaches and athletes must also register using the USA Gymnastics meet reservation system. The name of the clinic is "WI State Clinic with Mike Hunger Clinic".
A copy of your responses will be emailed to the address you provided.
