Kindly please declare an updated contact information so that the members of the committee can contact you for additional queries, updates, clarifications and/or expedite the verification processes of your application.

Incorrect filling of this form will greatly affect the process of your membership registration. 

The National Board of Trustees and the members of the Joint Committee will review your membership status. Their judgement is final and unappealable.    

Upon the approval of the board and the members of the joint committee, if you belong to a registered club or group and you are eligible to a Federation ID. Kindly coordinate with your club officials to process the payment/s; 

and if you registered as a Regular Member (Individual) after accomplishing this form kindly immediately notify any MFPI Official in your area to ensure the processing of your application.
