Invitation to participate:
You are being invited to participate in a research on-line survey on human pathogenic micro-organisms that is being conducted by the WG2 members of COST CA16110 HUPLANTControl under the supervision of Dr. Joël F. Pothier, researcher at the Zurich University of Applied Sciences, WG2 leader, and Dr. László Kredics, researcher at the University of Szeged, WG2 vice leader.
We would like your permission to fill out a survey that will help us to set priorities for more precise pathogen profiles.
Purpose & participant enrolment:
If you agree to participate, you will first be asked to provide some minimal information concerning you. You then will be asked to fill out an on-line survey about the importance of human pathogenic micro-organisms. This survey contains 11 questions and should take around 15 minutes to complete. We expect to have about 100 participants to take part in this survey. We especially rely on you for spreading further the information about this initiative.
IMPORTANT: Your answers will only be saved once you have reached the end of the survey.
Risks & benefits:
There are no foreseeable risks to participating in this survey. The direct benefits to you are minimal but we do think that this survey will make a contribution towards several Deliverables of the COST HUPLANTControl Action.
Your participation in this survey is completely voluntary. You have a right to refuse to participate without consequences or to discontinue your participation at any time without penalty.
If you agree to participate, you will be asked to provide a valid email. This is mainly to avoid multiple participation to this survey, and it will not be used to contact you, except if you request so, and will not be transferred further. Your email will not be linked to your name. Your answers will remain completely anonymous and will be only related to the country you are in, except if you include your name at the end of this survey. Once the data collection is complete, your name will be removed from the file that contains your responses to this survey. We may publish results of this survey, but the names of people or any identifying characteristics, will not be used in any of the publications. If you would like to be further informed about the results of this survey, please inform us at the end of the survey and we will inform you some time in the future.
If you have any questions about this research, you can contact Dr. Joël F. Pothier at email@example.com
and/or Dr. László Kredics at firstname.lastname@example.org
