Preorder form for uniform shirts
This is a pre-order form for uniform shirts for the 20-21 school year. Pre-ordering is not required, but families may choose to do so to save time at the front office. There is a purchase limit of 3 shirts per student, until further notice. Please fill out a separate form for each student. Shirts will be pulled and held for only 14 days, please plan to pick up at the front office during this time. Pre-orders that are not purchased within the 14 days will be returned to stock and sold to other students.
