This form is for students who do not wish to reserve a spot in the class and wish to withdraw from the aquaDuck's programme.

Please complete one Withdrawal Form per participant/student.

*Please submit this Withdrawal Form at least TWO (2) WEEKS before desired withdrawal date. Email, SMS, or verbal notices of withdrawal will not be accepted.

**During a student’s absence such as during holidays, if you wish to RESERVE your child's place in the class, please complete the Request for Holiday Proration form instead.
