To request your Certificate of Attendance for the DMV, please use the Google Form linked below

The Certificate of Attendance forms will be delivered to one of your classes within 48 hours of submitting this request. It is imperative that you thoroughly review all information regarding the laws and regulations for the Certificate of Attendance before completing this form.

Important: If you are deemed ineligible for the DMV Certificate of Attendance due to excessive absences or truancies, you will receive a formal notification of denial, which will also be delivered to one of your classes.

Nevada Laws on School Attendance and Driving

  • State Requirement: To apply for and keep a learner's permit or driver's license, students must regularly attend school. Truancy is unacceptable!

  • What is a DMV-301 Form: When you apply for your permit or license, you must submit a completed DMV-301 form to the DMV, signed & approved by a school official.

  • What It Does: Verifies that you have attended at least 90 percent of the school days in the current semester - i.e. you do not have more than 10 unverified attendance marks for the current semester. You must also not have 3 truancy marks in your attendance on separate days.

  • What if My Attendance has More Than 10 Days Absent or has 3 Truancy marks? If you were absent (truancy is not included in this), instructions are listed below to have your parent/guardian excuse your absences. If you are considered truant, you'll have to wait until the next school year to apply.

Habitual Truancy: The Consequences

  • What is Habitual Truancy? You are declared habitually truant if you have 3 or more truancy marks within a school year, on 3 separate days. You will not qualify for the DMV-301 Form for that whole school year.

  • Penalties for Habitual Truancy:

  • Turning 18 during License or Permit Suspension? You still need to complete the suspension period.

License Suspension: If your license or permit is suspended due to poor attendance, you must surrender it to the state of Nevada.

Reporting Absences: The Right Way

  • Parent/Guardian Responsibility:

  • Parents will use the Google Form on the school's main webpage to submit attendance notes.

  • No Paper Notes Accepted: If you have a doctor's note, upload it through the Google Form.

  • Student-Submitted Notes: Will be REJECTED! All absent excuses MUST be submitted by a parent or legal guardian. Students are not allowed to excuse their own absences!

  • If you were absent the whole day, have a parent submit the google form to excuse your absence. If you have an appointment during the school day, your parent will need to pick you up and sign you out. If you come back they will need to sign you back in.
