I do hereby waive, release, and forever discharge the JCC's and its officers, agents, employees, or executors, and all others from any and all responsibilities or liability from injuries or damages resulting from my participation in any activities or my use of equipment. I hereby agree to expressly assume and accept all risks of injury or death. I do hereby further declare myself to be physically sound and suffering from no condition, impairment, disease, infirmity, or other illness that would prevent my participation or use of equipment or machinery. The Katz JCC reserves the right to suspend or revoke program or membership privileges of guests or members whose behavior is deemed inappropriate or detrimental to the well-being of members or staff. Upon revocation of membership, the JCC reserves the right to retain any program or membership monies.
