OFA requires permission for young people to be filmed and/or photographed at OFA events. In confirming below, you agree that the young person's image can be used for:

• Publicity material and OFA information
• Newspapers and media outlets as appropriate
• The OFA website

Conditions of use

1. The permission given by signature above covers the reuse of the image(s) up to three years after the end of the student’s involvement in OFA activities.
2. In the event of use photographs of individual pupils or small groups of pupils, we will avoid using the full name (ie the forename and family name) of that child / those children in the accompanying text or photo caption. Please note, however, that the general media may use forenames and family names in their publications and/or broadcasts.
3. We will not include personal e-mail or postal addresses, telephone or fax numbers on video, on websites, or in any printed publications.
4. We may use group photographs or footage with very general labels, such as “a rehearsal session”.
5. We will only use images of players who are suitably dressed, to reduce the risk of such images being used inappropriately.
