The City of Los Angeles Housing Department (the City) wants to gather information from people living with HIV/AIDS in Los Angeles County regarding their housing and other supportive service needs. The City will use the survey results to make possible changes to the Housing Opportunities for Persons with AIDS (HOPWA) program to help improve the housing and supportive services for eligible people and their families.

Only people who have been diagnosed with HIV/AIDS and live in Los Angeles County should take this survey.

Information collected from this survey is anonymous. Incomplete survey responses may not be reviewed.

This survey will take about 30 minutes to complete.
