Pig Trails, Inc. Foster and Adoption Application
This serves as both an application and contract. Although you sign the application, it does not mean that you are guaranteed a pig. Upon adoption approval, you will sign one more form verifying that you are adopting the pig. Upon signing the adoption form, your signature on this contract will be considered a part of the adoption contract. All answers must be true and correct on this application as part of the legal adoption agreement.

This section contains all of the legal terms and conditions should you move forward and adopt the pig named above.
I agree to contact Pig Trails, Inc. if for any reason I have any issues with the pig.
I agree to ONLY return the pig to Pig Trails, Inc. should the adoption or foster not work out unless other arrangements are approved, in writing, by Pig Trails, Inc.
I agree to allow a Pig Trails, Inc. to visit my home and access my pig should it be requested.
I agree to surrender my pig to Pig Trails, Inc. should it be found I am breaking any of the legal conditions in this section or abusing/neglecting this pig in any way.
I agree to feed the pig at lease once a day (preferably two times a day) and to supply fresh water at all times.
I agree to never let the pig free roam outside of a fenced in yard and that, if not inside my house or fence, the pig will be on a harness and leash (a collar of any type is never allowed, a harness MUST be used). I understand that pigs are prey animals and are in danger from animals if not behind a fence.
I agree to never use my pig other for purposes of human companionship--for instance, that it will not serve as bait or food. If it is found I am abusing or neglecting the pig in any way, I will immediately surrender to Pig Trails, Inc.
I agree to never have the pig's tusks surgically removed.
I agree to take the pig to a licensed veterinarian for a yearly physical. I agree to worm the pig every 6 months and to keep any shots required by the state up to date. I also agree to provide vet care if the pig is sick or injured. I understand that Pig Trails, Inc. can check up on the pig's vet care at any time. If the pig isn't getting proper vet care, Pig Trails, Inc. has the right to remove the pig from my care.
I agree that I will spay or neuter my pig within 3 months of arrival should that be necessary and to never allow my pig to breed.
Under risk of perjury, I verify that no member of the household has been convicted of an animal welfare law violation, such as neglect, cruelty, abandonment, etc.
I agree to never use a collar, including choke-type collar, and to only use a harness on the pig.
I agree that I am taking responsibility of this pig to treat and be responsible for him/her as my own pet.
I agree to accept responsibility of the pig at my own risk and to release Pig Trails, Inc. from all liability arising from having this pig reside with me.
I agree to all financial responsibility for this pet at the date of my signed adoption contract, unless I am fostering (fosters are responsible only for food). Pig Trails, Inc. will not be responsible for any damages or expenses, veterinary or other, during my ownership of the pig.
I agree that in the event the pig becomes lost or dies I will immediately notify Pig Trails, Inc.
I agree to notify Pig Trails, Inc. of any change in contact information (address, phone number, email address, etc.) within 48 hours of change.

I agree and understand that the pig's known background and medical history have been disclosed to me. I understand that Pig Trails, Inc. has made no representation concerning the health, condition, training, behavior, or temperament of the pig.
I agree to permit Pig Trails, Inc. to make inquiries about and enforce any of the above conditions and requirements at any time after the adoption or foster. This can include home visits or contacting my veterinarian. I UNDERSTAND THAT FAILURE TO COMPLY WITH ANY OF THE ABOVE PROVISIONS WILL RESULT IN FORFEITURE OF THE PIG AND IMMEDIATE RETURN TO PIG TRAILS, INC.
I understand that by voluntarily signing the final adoption agreement, I am entering into a legal and binding contract with Pig Trails, Inc. I understand that, in the event of violation of my adoption or foster agreement, I will pay $500.00 to Pig Trails, Inc. as a fine for breaking the contract and will immediately return the pig to Pig Trails, Inc.
