ACS Transportation Request Form
Complete this form to request school bus transportation for your child. There will be 3-5 school days for processing.
EC students needing transportation as a related service will need to meet with their IEP team to arrange transportation.
Every new request completely overwrites every previous request. Please make sure both AM and PM requests are complete. If your address has changed, you will need to contact your student's school to update that address before requesting a bus.
