■ 個人情報の扱いに関して
1. ご提供いただいた個人情報は、Art Collaboration Kyoto 実行委員会（以下「実行委員会」といいます）のプライバシーポリシーにもとづき、事務局業務の委託先である一般財団法人エーシーケー（以下「事務局」といいます）が管理いたします。ご提供いただいた個人情報は、事務局のほか、本事業の関係者（ACK運営のためのその他の業務委託先など）に提供することはありますが、それ以外の第三者への提供はいたしません。なお、下記 iv) の調査・分析にあたっては、回答者個人が識別されない方法で集計いたします。

2. Art Collaboration Kyoto（ACK）に関して実行委員会および事務局に提供された個人情報の利用目的は、以下のとおりです。
   i)    ACKのイベント等のご案内（次回以降開催されるイベントを含みます）のため
   ii)   上記イベント等へのお申込受付および実施に関する連絡のため
   iii)  アンケートの送付のため
   iv)   いただいたご回答の集計、ならびに今後の企画のための調査・分析のため
   v)    お問合せへの対応のため
   vi)   本事業及びこれに関連する事業の推進のため

■ ニュースレターの配信について
ACKのご招待者様には、不定期でACKに関するご案内をお送りいたします。ご希望によりいつでも配信停止（登録解除）が可能です。ただし、システムの仕様上、メーリングリストのご登録を解除された方には今後ご案内をお送りすることができませんので、あらかじめご了承ください。特にお申し出がない限り、ご登録いただいたゲストには今後もご案内をお送りいたします。

■ Handling of Personal Information
1. The personal information provided in this form will be managed by the General Incorporated Foundation ACK (“Secretariat”), which has been commissioned to handle secretariat operations, in accordance with the privacy policy of the Art Collaboration Kyoto Executive Committee (“Executive Committee”). The personal information provided in this form may be shared with parties involved in this project (such as other contractors for the ACK operations) in addition to the Secretariat, but will not be disclosed to any other third parties. Please note that in conducting the surveys and analyses described in item iv) below, personal data will be aggregated in a manner that does not identify individual respondents.

2. The purposes for which personal information provided to the Executive Committee and the Secretariat regarding the Art Collaboration Kyoto (“ACK”) will be used are as follows:
i) To provide information regarding events and other activities for the ACK VIP invitees (including events to be held in the future)
ii) To accept applications for the above events and other activities and to communicate regarding their implementation
iii) To send out surveys
iv) To compile the responses received, and to conduct surveys and analyses for future events
v) To respond to inquiries
vi) To promote this project and related initiatives
vii) For other matters incidental or related to the above

■ Newsletter Distribution
We will send the ACK Newsletter to the ACK invitees from time to time. You may unsubscribe from the Newsletter at any time. However, please note that due to system limitations, we will be unable to send the ACK invitations to those who have unsubscribed from the mailing list. Unless you request to unsubscribe, we will continue to send the Newsletter to registered guests.
