12/6 Kobe Citizens Chat Cafe (CCC) 申し込みフォーム/ Application form 
このフォームは、神戸市外国語大学が主催するKobe Citizens Chat Cafeのお申し込みフォームです。
一回のフォーム送信につき、お一人様、一度のセッションの申込みができます。

イベントの開催日時は以下の通りです。全日土曜日の開催です。
2025年：６月７日、２８日、７月５日、９月２０日、１０月１１日、１１月８日、１２月６日
2026年：１月２４日
本フォームは12月6日へのお申し込み用です。
今回から、イベント毎に違うフォームにてお申し込みしていただきます。
次回のお申し込みは、6日のイベント終了後に新しいフォームを共有致します。公式ラインまたは公式インスタグラムをご確認ください。

複数人参加をご希望の際は、お手数ですがその都度フォームをご入力いただき、お送りください。
※お子様の保護者様なども参加される場合はその人数分フォームをお送りください。なお、見学や付き添いのみのご参加はできません。ご了承ください。

※当フォームを送信されますと申し込みが確定します。
フォーム送信の後にキャンセルをご希望の際は、Eメールまたは公式ラインにご連絡ください。席に限りがあるため、無断キャンセルはお控えください。大学へお電話で直接問い合わせする事はお控えください。

This form is for applying for Kobe Citizens Chat Cafe hosted, hosted by Kobe City University of Foreign Studies.
One person may register for one session per form submission.

The events will be held as following dates on Saturdays.
2025: June 7, 28, July 5, September 20, October 11, November 8, December 6
2026: January 24
This registration form is for the session on December 6th.
From this session, you will apply via a different form for each session.
You can apply for 12/6 session by using the new form that will be provides through the official LINE or instagram. Please check those message!

If you wish to attend more than one session, please fill out and submit the form each session separately.
※If you would like to register multiple participants, please submit the form separately for each person. 
Please note that we are unable to accept visitors or chaperones without registered participants.
