Request for Change of Instructional Model
If you would like to request a change of instructional model please fill out this form. (Please fill this form out once for each child you for whom you would like to request a change in instructional mode.)

A change of instructional model for students in Kindergarten through Grade Five may result in your child being assigned to a different homeroom teacher. We can not accept requests for specific in-person days for students moving from the Remote Academy into hybrid learning.

Any changes to your child's mode of instruction can take up to four weeks to process according to DESE guidance.
