Patient information sheet:
1. What is the purpose of the project?
This project is towards the completion of a dissertation for an undergraduate degree in Sports Rehabilitation at the Universtiy of Hull.
2. Why have I been invited to participate?
You have been invited to take part as you have been identified as someone who is living with a lower limb amputation and fits all the inclusion criteria for this project.
Inclusion criteria: You are aged over 18 years, male or female, living with a lower limb amputation for longer than 6 months, have an amputation related to any cause, and are able to give your full informed consent
However, if you fall into any of the exclusion criteria below, please do not complete this survey:
Exclusion criteria: You are younger than 18 years, have a partial foot or toe amputation only, have had your amputation for less than 6 months, or you are unable to give your full informed consent.
3. Do I have to take part?
No. This project is completely voluntary, meaning you do not have to take part if you don’t want to. You can stop being part of this project at any time, and all information gathered from you will be immediately deleted and not used any further in this project, and no further data will be collected.
4. What will happen if I take part?
If you choose to participate in this project, you will complete an online survey that lasts around 10-15 minutes. This will be sent out electronically via email or WhatsApp. This will be completely anonymous, and no private details will be required. The survey will be looking at the physical, mental and social health of the participant
5. Are there any benefits to taking part?
Participating in this research will help gather information that can be used to help improve a person's experience of their amputation in future
6. What information will be collected, and what will happen to the information provided?
The information I will be collecting will be based around your social, mental and physical health post amputation, as well as looking at the help you were provided and how it benefited your transition as well as the help you wish you had available to you that would make this transition easier.

8. Data protection?
When collecting personal information and data, this will be stored on a University-approved password-protected storage system, which is only accessible by me. Once I have finished with the relevant data, all information will be destroyed by myself, deleting all personal information from the storage system
