A schedule may be changed only for the reasons listed below. Do not submit requests that do not meet one of the following criteria. Circle one.
1. Student already passed the class.
2. Student must make up a failed required class.
3. Student must be enrolled in a class required for graduation this year.
4 Student has not taken the prerequisite class.

Students may not request a different teacher for the same course.
Students may only submit one change form. All other forms will be denied.
Students are not to report to the counselor's’ office with the form.
Students will be called to the office if needed.
