Maryland AT Reuse Program Application   
  The Maryland High-Tech Assistive Technology Reuse Center (MATR) is a program of the Maryland Department of Disabilities, AT Program.

MATR provides high-tech assistive technology to individuals at no cost for as long as they need it with the expectation that it will be re-donated when the user is finished with the device.

By completing this application, you are requesting access to a gently used, donated device.

**MATR program deals exclusively in Assistive Technology. If you are in need of medical equipment 
(walkers, wheelchairs, rollators, etc.) please contact Maryland Department of Aging's reuse program.**

**MATR does not accept computers/tablets or smart phones, therefore donations of this type of equipment is rare**
