Number 2 Student Registration Form
This is the second registration form. Families who have not completed a registration form yet should complete one form for each child. Some families may have changed their minds since the board approved an amended version of the model; if you would like to change your response, please complete a registration form and we will delete your previous response. One registration form only for any child you wish to change to a different model.
DEADLINE is Friday, 8/14/20 at 6:00 PM. Schools will contact families starting on Monday, 8/17, for any student who does not have a registration form completed.
