Everyone must complete this mandatory COVID-19 Screening form within 24 hours prior to entering Milliken Gospel Church premises.  ALL individuals attending MGC activities, including staff, children and any visitors who enter our church building must be screened prior to entering the premises.

All personnel may only enter Milliken Gospel Church premises if they have passed the screening. Any person who enters Milliken Gospel Church must continue to follow all public health and workplace control measures, including wearing masks, maintaining physical distancing, and practicing hand hygiene.
