Our office can provide a Letter for Event Program or Journal, a Certificate of Recognition, a Personal Certificate (birthday, wedding/anniversary, or retirement), an Adjourning Motion or In Tribute, or a Certificate for another occasion.
If no organization, please use the name of the honoree. For an adjourning motion, use the name of the person requesting the letter.
When will the actual event take place? Please use the format: mm/dd/yyyy
Requests for letters to be published in an event program or journal will have a print deadline in advance of the event. Enter that date here. If this does not apply to your request, enter the event date, same as in the previous item.
