2019 Whole Health application form
Participants in Whole Health PRB will receive at no cost:

A Community Supported Agriculture (CSA) food share consisting of fresh produce provided weekly by Happy Dancing Turtle.

Hands-on cooking classes with a University of MN Extension Educator with take home recipes and other information on the fruits and vegetables you received.

Requirements:

Commitment to participate in this initiative from June through September of 2019.

Pick-up of CSA share every Thursday at the Family Center in Pine River between 12:00 and 3:00. (Alternate arrangements may be possible if arranged ahead of time.)

Participation in at least 4 out of 6 cooking classes. Cooking sessions will be held locally and several will be offered during the season.

Participation in a pre- and post-survey.
