Please complete this form if you would like to enroll your child in a 2nd quarter Oakschool course. In completing this form, your child's Instructional Fund account will be charged $75 per course. Do not submit this form if your child's Instructional Fund account cannot cover the cost of the requested course/s. Oakschool courses cannot be paid for out of pocket. There are
no refunds once this form is submitted.
Oakschool courses have limited space. Once the enrollment window is closed, Oakschool teachers will send a parent/guardian email alerting you of your child's enrollment or waitlist status. The completion of this form does not guarantee enrollment in Oakschool courses. Your student's instructional fund account will only be charged if they make it on the enrollment list. The account will not be charged if they are on the waitlist.
