Please complete one survey PER STUDENT by January 6. If you have more than one son attending SLUH, please fill out a survey for EACH student. You will select EITHER option 1 OR option 2 below.
The week of January 4 will be online to provide a buffer from Christmas break and allow ample time for shifting furniture, signage, and equipment. Students may return to campus for in-person classes beginning on Monday, January 11. Please see details about this next phase of reopening here:
https://www.sluh.org/coronavirusIf you select full-time online classes, you will be committing to this option through at least Tuesday, January 19. Moving to in-person classes will require a formal five school day notice to Mr. Tom Becvar at
tbecvar@sluh.org. Students not providing this notice will be sent home due to the spacing planning required for classrooms.
In the course of the semester, students can move to online coursework as needed, but they will then remain online for at least five school days. However, any student who is required to temporarily quarantine at home can return to in-person classes AFTER he is medically cleared by both his physician and SLUH to do so.
Both online and in-person academic programs will be synchronous and of equal rigor. Students at home will be expected to be on time, prepared, in school dress, and using video. Students at school will be required to wear an approved face mask and remain distanced (six feet) whenever possible. A minimum of three feet distancing will be required at all times.
We understand that these are complicated times and challenging decisions. Feel free to contact me at
igibbons@sluh.org to address any questions.
Thank you for your timely response by Wednesday, January 6. Please DO NOT provide health information below. If necessary, our health team will contact you based the results of this survey.
Please continue to follow all public health directives and stay safe.
